Criteria | Requirement |

Roles and Responsibilities Of KHPT Community Facilitator
As a Community Facilitator, you will:
Support digital health enrollments under the Ayushman Bharat Digital Mission (ABDM).
Coordinate and guide field facilitators to complete ABDM-related tasks.
Collaborate with Urban PHC (UPHC) staff and medical officers for smooth implementation.
Solve on-ground challenges at the UPHC level.
Conduct monthly meetings, collect reports from field staff.
Plan and conduct training programs for team members.
Ensure timely completion of digital entries and project targets.
Maintain the quality of work and regularly report progress.
Share successful case stories and attend review meetings.
Take up any additional responsibilities given by the District Program Officer.
